Case management services for children with developmental disabilities 

 PROGRAM OVERVIEW: 

• We support children, youth, and their families to get the supports they require to reach their goals
• We are client and parent-led in the supports and services we provide
• We work within the city of Toronto helping families access community organizations,developmental sector agencies, schools, and medical teams to support a whole person approach.

ELIGIBILITY: 

• Diagnosed with a developmental/intellectual disability
• Birth to 18 years old
• Reside in Toronto region (M postal code)

WHAT TO EXPECT FROM FST OPTIONS SERVICES: 

• we provide consultation
• we support with advocacy, resource referrals, and community engagement
• aftercare and counseling services

Workshops and Clinics 

• Funding for camps/respite
• Service navigation
