This program is suitable for you if you:

• Can participate in goal setting and planning, with or without support
• Are open to involving family members, caregivers, or a support team in your planning (if needed)
• Have access to a computer or Both a tablet and a cellphone
• Are willing to reflect on past experiences to help shape your future goals.

This program is not suitable for you if you:

• Need in-person planning facilitation.
• Communicate non-verbally and do not use assistive communication tools.
• Are Deaf or blind, and do not have the necessary support to engage with online planning tools.
• Prefer a fully guided approach rather than self-directed planning.
• Only have access to a cellphone for online participation.
• Have significant behavioural challenges that make structured planning sessions difficult.

Ratios: 1 staff to 1 participant

Program hours:
12 virtual planning sessions within one year.
Between Monday-Friday, 9-5.
To be scheduled according to your schedule.
