      As a family, it helps to think about your goals and vision for life after high school.

      Person-directed planning can help you get started.

      Learn more about resources, programs and services and be prepared to be an active partner in the process. There are lots of resources on ConnectABility.ca like this resource list and these guides to transition planning.

      Employment supports and/or continued learning programs can help you on your path. Here’s an overview of some options.

      Talk to other families. You may be able to connect to a family group through your local Community Living organization and/or your local children’s treatment centre. The Connected Families forum on ConnectAbility.ca is a virtual way to do this.

      Ask your school team for a Transition Planning meeting.
