Community Networks of Specialized Care

The purpose of the Community Networks of Specialized Care is to enhance service to adults with a developmental disability who need specialized care for co-existing mental health and or/ behavioural issues in Ontario. There are 4 networks: Northern Region, Southern Region, Eastern Region and Central Region. http://www.community-networks.ca/

Individualized Funding Library

MyCommunityHub is an online registration and payment space for activities, classes, programs, workshops, respite and camps offered by Developmental Service Agencies across Ontario. http://mycommunityhub.ca/

Fee for Service Program Directory

The directory only lists programs that are not in MyCommunityHub and has been compiled for information purposes only.
This directory contains Community Participation Support, Caregiver Respite and Person-Directed Planning Support listings for transitional age youth and adults that may be purchased using Ministry of Children, Community and Social Services “Passport” funding or personal funds. Cost varies based on an individual’s support needs and program activities. Families/individuals are responsible for assessing each program to determine whether it meets their needs. https://connectability.ca/directory
